The evidence is much more complicated.

In other words, anxiety may sometimes be less about possessing a super-sensitive internal sensor and more about what happens when bodily information attracts attention and is interpreted negatively.

You can notice a sensation without knowing what it means

This distinction is easy to miss.

You can know: "My heart is beating strongly."

That is a sensation.

Then the mind may add: "This means something is wrong."

That is an interpretation.

The interpretation might be correct. Physical symptoms sometimes do require medical attention.

But the sensation itself does not automatically tell you which explanation is correct.

Physical anxiety can become self-reinforcing.

A 2018 systematic review and meta-analysis found stronger catastrophic interpretations of bodily sensations in people with panic disorder than in healthy participants and people with other anxiety disorders.

A more recent 2024 systematic review of the cognitive theory of panic disorder concluded that catastrophic misinterpretation remains an important part of the evidence, while also finding that the overall theory is more complex than a single mechanism.

That distinction matters: not every racing heart is panic, and panic is not explained by one thought alone.

The body sensation is real even when anxiety is involved

People sometimes hear `it's anxiety` as though they have been told the symptom is imaginary.

That is a false distinction.

A physical sensation can be genuinely uncomfortable while being produced, amplified, or maintained partly by psychological stress.

Psychological does not mean fictional.

You may have noticed that your mind sometimes feels calmer before your body does.

The difficult conversation ends, but your shoulders are still tense.

You realise the feared email was harmless, but your heart is still beating quickly.

That does not mean you are secretly still anxious or that something has gone wrong with regulation.

Physiological changes do not need to reverse instantaneously when a thought changes.

Different body systems operate on different timescales, and continued monitoring of whether you are `calm yet` can itself keep attention fixed on the sensations.

When a sensation worries you, checking it makes intuitive sense.

Is my heart still racing? Is that pain still there? Am I breathing normally yet?

But repeated monitoring gives the sensation more attention.

There is no single technique that fits every bodily anxiety response.

Start by deciding what kind of problem you are dealing with.

2. Reduce unnecessary physiological arousal when it helps

If you are breathing quickly, physically tense, or restless, a body-based technique may make the experience easier to tolerate.

That could mean slower breathing, walking, releasing muscle tension, or using a grounding technique.

The aim is not to force the body to become completely calm.

A small reduction in arousal can be enough to give attention more flexibility.

3. Examine the story attached to the sensation

Sometimes the body is only half of the loop.

Sensation: "My heart is beating fast."

Story: "I'm going to lose control."

Once you can see both parts, cognitive reframing can help you evaluate the second one.

Ask what you actually know, what you are predicting, and whether you have experienced similar sensations before without the feared outcome occurring.

The goal is not reassurance. It is a more accurate interpretation.

4. Sometimes the goal is to tolerate the sensation, not remove it

This may sound counterintuitive.

If you become afraid of anxiety sensations themselves, making every exercise about getting rid of them can teach your brain that those sensations really are dangerous.

In CBT for panic disorder, interoceptive exposure may deliberately produce feared sensations — under appropriate conditions — so that a person can learn something new about them.

A 2025 scoping review of 132 studies found that interoceptive exposure has been used across several mental and physical health conditions, with the clearest evidence coming from multicomponent CBT approaches.

This is not an invitation to provoke severe symptoms yourself. Clinical exposure exercises should be matched to the person's health, fears, and treatment plan.

The broader lesson is that regulation does not always mean eliminating every uncomfortable sensation.

However, a 2022 meta-analysis of 55 studies found no evidence that higher anxiety was associated with greater objective accuracy in detecting cardiac signals.

Even neuroimaging evidence remains less tidy than simple nervous-system explanations suggest. A 2026 systematic review and meta-analysis found no statistically significant pooled differences during interoceptive tasks for anxiety disorders, although qualitative findings suggested possible altered insula activity and the researchers highlighted substantial methodological variation.

The evidence therefore points away from one simple problem such as `people with anxiety feel their bodies too accurately.` The more useful model includes attention, interpretation, learning, bodily arousal, and the context in which sensations occur.

When anxiety feels physical, immediately trying to debate the thought is not always the most useful first move.

If your body feels highly activated, you can begin with breathing, grounding, somatic tapping, or bilateral stimulation.
