It connects the trigger, thought, emotion and body sensations before separating observable facts from interpretation. You do not need to challenge the thought immediately. This exercise is a starting point for deciding whether to reframe, solve a problem, seek reasonable information or leave a question open.

How to use the worksheet

  1. Trigger / situation

    Record what was happening immediately before your emotion shifted.

  2. First thought, image, or meaning

    Capture the first words or image, even if incomplete or uncomfortable.

  3. Emotion + intensity

    Name the feelings and rate them 0–100.

  4. Body sensations

    Notice sensations without using them as proof of the thought.

  5. The hot thought

    Choose the sentence that best explains the strongest emotion.

  6. Camera test: what is observable?

    List what a recording or neutral witness could confirm.

  7. What did my mind add?

    Separate predictions, assumptions and mind reading from those facts.

  8. What deserves attention next?

    Decide whether the thought needs examination, action, information or time.

Their motive, that I caused it, and that the friendship is changing.
What deserves attention next?
No action is needed tonight. If the distance continues, I can ask directly instead of guessing.

Tips for getting more from the exercise

  • A thought can be an image or an implied meaning, not just a sentence.
  • Capture the thought before trying to make it sound reasonable.
  • You do not need to examine every passing thought. Choose one that affects what you do.

What this worksheet cannot tell you

This sheet helps identify a thought; it cannot establish another person’s motive or diagnose the cause of physical sensations.
